So a little update.
Today I got some TruPix and Xpres sub paper, along with matching profiles to match my 3110dn.
I tested all papers, CIE, TruPix and Xpres papers (the correct way up! with the corresponding ICC profiles for each).
All 3 had visible horizontal lines as before (Very visible on one brand, not so much on another, but still noticeable, CIE same as before.)
I then tried some regular tesco blue stripe paper with the same CMYK test image id been using. This image came out perfect, no lines, perfect block colours.
This tells me the printer itself is perfectly capable of printing block colours, the only difference in my testing is that the one that printed out perfectly just used the standard windows print settings, no ICC or photoshop involvement.
The plot thickens!

I had an issue with one particular image a while back, the printer would add a horizontal red line which did not exist in the image. Eventually changed from ICC to Power Driver which cleared the problem. No logic here but it worked. Skipped the printer when the new model arrived and had no problems since.

I tried the power driver this morning after your post wayupnorth. Unfortunately it didn't resolve my issue, banding still there.
I also tried CityInkExpress' suggestion and changed the paper type to Inkjet Plain Paper so that it would flood the paper to hide the lines, this actually had the opposite effect and made the lines A LOT worse, so much so that the cyan was then worse than the magenta.
Black and yellow remain banding free throughout all testing.
If I use CS6 and turn off all colour management, in both CS6 and the printer, i get smooth blocks of colour, this again shows that the printer and ink are capable of producing a perfect result, i tried this on both normal A4 and 3 types of sub paper, all came out perfect...apart from the colour accuracy obviously.
Ive also re-installed CS6, The printer and all ICC profiles from scratch.
My final 2 tests are:
To try printing with colour management from another laptop via CS6
To try CS5 or PS CC to see if there is any difference.

Ok so Adobe PS CC produced the same magenta white banding, so no good.
HOWEVER!!!!
CoralDraw 8 produced perfect magenta with CIE, xpres and TruPix paper WITH their corresponding ICC profile. I have attached one photoshop print and one corel draw print, both using 100% magenta.
So Photoshop CS6 is definitely the problem, not the printer, now I just need to figure out how to fix it!
Stay tuned.

This morning I have installed;
CS4 and CS5.1 as well as using the Adobe removal tool to completely wipe and re-install CS6 (making sure to remove all traces before re-installing)
All 3 still produce white banding when PS manages colour
All 3 produce smooth block colours with no banding when printer is set to manage colours.
The issue it seems is crystal clear, PS cannot use the ICC profiles I have, it used to, but not anymore.
Another test this morning using coraldraw for completeness with ICC profiles applied - low and behold perfect prints!
In my opinion this extensively proves it is a software issue, not a hardware issue.

I use Photoshop CS5 and only have banding problems when a nozzle is partially blocked, usually the magenta one.
I am using CIE inks, an ICC profile supplied by them, and the 'Anne' type paper. Photoshop is set to manage colours. Printer driver is set to plain paper and high quality.
Nothing out of the ordinary other than my printer driver is also set to print using EMS as I was having initial troubles with the driver status dialogue not appearing sometimes when starting to print. This may have just been down to me using Vista though.
I have had problems before with PS not printing smooth gradients correctly and thought this to be a problem with the way the Ricoh driver handles dithering.

Probably worth creating a gradient that includes the spot colour and printing. Have you checked the gamut in PS. If you are out then the ICC profile may have problems resolving this. The printer version will probably have less points in the profile and may respond differently.

So I took a punt and got an Epson wf7110 with ciss.
As expected, this did not fix the issue, horizontal banding in magenta and black. I now have a very large and expensive paperweight if anyone wants it?
I've also done a complete teardown and reassembly of my ricoh print head, will get a new thread up in case anyone else wants to do a manual clean of their sg3110
